HUMAN ERROR CAUSED YAKOWA, AZAZI’S CHOPPER CRASH –INVESTIGATION

Human error was responsible for the Navy chopper crash that killed Kaduna State Governor Patrick Ibrahim Yakowa and former NSA General Andrew Owoye Azazi, an investigation into the crash said.
The chopper came down near Ogbia Creek in Bayelsa state on December 15, 2012 shortly after takeoff.
The naval headquarters in Abuja yesterday said the outcome of the investigation done by the air force and the manufacturers was out but the manufacturers of the engine of the chopper will make further assessment.  
Director of Information Commodore Kabir Aliyu said investigation was carried out by the Accident Investigation Board (AIB). “The investigation which was conducted by aviation experts in collaboration with the manufacturers, Agusta Westland has stated that the cause of the crash could either be human error or material failure or a combination of both,” he said.
He said “an independent police report has equally ruled out sabotage as a possible cause of the crash.”
